The Devonport Dry Eye Clinic uses this simple survey to give you and us a benchmark to where your Dry Eyes are Bothering you. It takes into account your symptoms, what taske you perform that are affected and the environmental conditions that may affect your eyes.

"0 - 12" - Normal"13 - 22" - MILD"23 - 32" - MODERATE"33 - 100" - SEVERE
